This program is a Membership Discount Plan and is not dental insurance.

Membership benefits are valid for use only at Smile Cabarrus Family Dentistry.

Benefits are non-transferable and may not be applied to services provided by any other healthcare professional or facility.

Benefits expire at the end of the 12 month membership year and will be renewed upon receipt of the next year’s annual membership fee.

Payment for dental services are due the day of service.

Benefits may not be used in conjunction with a dental insurance plan, discounts, coupons or patient financing programs (i.e. Care Credit, Cherry Financing, Wells Fargo, payment plans, etc).

Membership fees are non-refundable and non negotiable.

Any unused annual benefits will be forfeited.

This membership cannot be used in conjunction with any third party liability insurance (home, auto, workers compensation).
